Please ensure the following is done before turning in a sports physical:
- Doctor has signed AND dated the physical
- Doctor has checked cleared or not cleared
- Parent and student have both signed and dated the pre-medical history form.
All copies must be submitted digitally and none will be accepted in person.

Big Teams Registration

Here is the link that applies to current and future sports, this link goes through your Configio account. If you do not have one, please create one. If you have one, this should be easy for you. This account covers Athenaeum, clubs, and anything else school related.
